Office Promotions

The Patient Referral Program is designed to reward our valued patients with great prizes.  This is our way of saying Thank-You for your referral.  The following shows you how the program will work.

 

One referral: $25.00 gift card to Mazzios, Tinsletown Movie Theater, or Exxon Gas Card.

 

Two referrals: $50.00 Exxon gas card, professional strength Crest White Strips, or an iPod shuffle.

 

Three referrals: $75.00 Exxon gas card, Sonicare Flex electric toothbrush, or custom made bleaching trays.

 

We will also be holding a grand prize drawing on June 1 & Dec. 1 of each year for each person who makes three or more referrals.  You can increase your chances at winning the grand prize by having your name entered with each referral you make after your third one.  The program will run each year from June 1 - Nov. 30, then begin again on Dec. 1 - May 31.   So send your family, friends, and co-workers and be sure that they tell us you sent them!  Good Luck!

The program is limited to one reward per patient.  The number of referrals will be added together and rewards will be given out the week of June 1 & Dec. 1 at which time the drawing for the grand prize will be held.  The number of referrals will start over on June 1 & Dec. 1 of each year.
